Treatment is divided by the cause of symptom into internal and external treatments. Internal hemorrhoids do not have cutaneous innervation and thus can be destroyed without anesthetic. Internal hemorrhoids are classified by symptom. Grade I hemorrhoids only bleed. Grade II hemorrhoids prolapse and reduce spontaneously. Grade III hemorrhoids require manual replacement, and grade IV hemorrhoids are permanently prolapsed.

There are numerous methods whereby to eliminate internal hemorrhoids. These methods include rubber band ligation, infrared photocoagulation, sclerotherapy injection, laser ablation, Lord dilatation, carbon dioxide freezing, and surgical resection. With the sole exception of surgical resection, all of these methods are considered as non-operative procedures and it is highly recommended that these be the first methods of treatment used for grade I or grade II hemorrhoids that have not responded to conservative therapy.

When in the hands of an experienced physician, many third- and (sometimes) fourth-degree internal hemorrhoids can be treated non-operatively. All of these conservative treatment procedures have approximately the same efficiency rates as surgical procedures, when used by an experienced professional. Rubber band ligation is the most common procedure performed in the United States because it is the one that is most commonly taught in medical training programs. Lord dilatation hardly ever used in the United States, and many American colorectal surgeons denounce it because it is basically just an unrestrained disruption of the sphincter mechanism.

Sclerotherapy can be adequate treatment for early-detected internal hemorrhoids. Cryotherapy and sclerotherapy are not used very often today. Most experienced surgeons use just 1 or 2 techniques exclusively.

Hemorrhoid relief cure can be found in over-the-counter creams or suppositories. One may also find relief and cures in herbal or natural form. Highly effective relief comes from warm sitz bath soaks 3-4 times a day. Topical anesthetics and suppositories also help to relieve pain.
